Life Care Planning Workshop
Thursday August 26 at 10am in the Activity Room
Life Care Planning is important for Seniors. Arizonans can make their choices known about who will manage their medical affairs in the event of an emergency. The Attorney General’s (AG) website provides a package with a Health Care Power of Attorney (POA), a Mental POA, a Living Will, and a Prehospital Medical Directive (Do Not Resuscitate).
Villages will hold a workshop on August 26th at 10am in the Activity Room, where volunteers can help residents fill out the forms.
Before the workshop, you should review the package for all the information and decisions you need to make, or read this summary. Bring all your information to the workshop where volunteers are available. You may also fill out the forms online, print, then bring them. Someone can witness your signatures.
Signatures can be witnessed at the workshop. Mail completed forms to the Arizona Secretary of State's office.
If you do not have a computer, the package is available in the Activity Room. No signup is required. Just show up on the 26th.
